Insurance Cards

If you have a new insurance card, please text images of the card (front and back) to:  (435) 295-4533.

Having insurance information up to date in our system is the best way to insure claims are processed in a timely manner.  

No Show and Cancellation Policy

We understand that life can be unpredictable.  To ensure fair access for all who seek care at our practice, we require at least 24 hours notice for appointment cancellations.  This policy includes, but is not limited to: sickness, inclement weather, scheduling conflicts, and transportation issues. Telehealth can be a great option if you are unable to keep an appointment due to one of these issues.  

If you miss a scheduled appointment, it will be considered a no show and a fee will be assessed as follows:  

  • For the first no show in a calendar year, you will be charged a $50 fee.
  • After the first occurence, no shows will be charged the full cash rate of the scheduled appointment time.
  • If you are more than 15 minutes late or cancel the same day, there will be a fee and you will be asked to reschedule.

New Patients and Deductibles

New patients with a deductible that has not been met will be asked to pay $100 toward their appointment at the time of service in addition to the $50 deposit that was taken when scheduled.
